For all counties that are a part of the Dental Society of Western PA

The following resources are available to anyone in category 1A in all counties:

Centerville Clinics

Centerville Clinics is a Federally Qualified Health Center, and we received the Moderna vaccine in December 2020, and will continue to receive it in 2021. You will have to travel to the site assigned to you. We have 11 medical and 5 dental sites, however, the vaccine is presently offered in 3 locations: Washington, Centerville (Fredericktown), and Uniontown, PA.

We are presently vaccinating anyone in Category 1A, which includes Dentistry.
*Please send all information to the DSWP as well, in case the county asks for it.

The process is simple:

  • go to our website: www.centervilleclinic.com
  • click on the “Important Covid-19 Vaccination Information” link
  • read the posted information
  • follow to the bottom and click on the link: “Centerville Clinics, INC-Vaccine Survey”
  • fill out the 5 required fields (name of organization, contact person, contact email, contact phone #, and # of eligible Category 1A hopeful vaccine recipients)
  • Submit

You will be contacted to schedule when the doses are available.

COVID-19 Vaccine for Non-Affiliated Health Care Workers

Effective January 6, 2021 AHN facilities will designate and utilize at lease 10% of each vaccine shipment received for vaccination of Phase 1A non-hospital-affiliated health care personnel, including Emergency Medical Services (EMS) personnel.

Please do AHN pharmacies, hospitals, or call centers to schedule an appointment.

As a reminder, the vaccine is still in extremely limited supply. How quickly we can provide vaccinations to non-hospital-affiliated health care personnel relies on the speed of production from manufacturers delivering to the state and the state distributing the vaccine to our facilities.

We will also continue to move through vaccinating our own employees as we vaccinate non-affiliated health care workers. Please note that we will follow the same vaccine prioritization schedule with AHN and non-affiliated HCW’s.

As of January 6, AHN is currently only vaccinating health care workers in Phase 1A-1. This group includes acute care inpatient personnel directly caring for COVID-19 patients (hospital, urgent care and EMS workers).

We will start to immunize ambulatory healthcare workers the week of January 18.

How to schedule for your vaccine: If you are eligible to receive the COVID-19 vaccine in the Phase 1A group and would like to request an appointment at an AHN facility, please contact us by email: covidvaccine@ahn.org

Please include the following information:

  • The names and best phone numbers of health care workers requesting vaccination
  • The job role and setting of each worker (patient-facing vs back office, hospital, office, etc.)

AHN staff will call or email you back to schedule.

We will also respond to general questions about COVID-19 vaccination.

Vaccine Scheduling at AHN

We are currently using vaccines requiring a second dose either 21 or 28 days after the first dose. When scheduling your first vaccine, please be aware of your schedule for the subsequent 3-4 weeks so that you can schedule your second dose in the proper time frame. Upon receiving your first dose of the vaccine we will issue a COVID-19 vaccine reminder card that provides a date for a return appointment for the second dose of vaccine.

If you are eligible in the Phase 1A group and would like to go to a pharmacy for your vaccination, names of pharmacies and location participating in the vaccination program are listed at the Pennsylvania DOH website.

The Allegheny County Health Department is seeking information from healthcare organizations to assist in planning and allocating the limited amount of COVID-19 vaccine being received. An eligible organization of facility must be phyically located within Allegheny County. It does not matter if the employees of that organization or facility do not live in the county.

Organizations which have employees that fall into the Phase 1A category are asked to have a single point of contact. That contact should complete the COVID Vaccination form through the county’s Online Support Center. The process requires the creation of an account, and then the user may select the options for Request Services, Health Department and finally, COVID Vaccination Request, to submit the information for their respective organization. Solo practitioners may also complete the COVID Vaccination Request form through the county’s Online Support Center.
